for current SPHS students

 

TWO STEP PROCESS to submit signed Transcript Release Form:

PARENTS: login to your Parent Portal account to sign your senior’s transcript release form.  “Senior Transcript Release Form” in the “Forms” sections. This gives SPHS permission to send your student’s transcript to any colleges and universities to which they are applying.

STUDENTS: login to your Student Portal account to sign your transcript release form.  “Senior Transcript Release Form” in the “Forms” sections. This gives SPHS permission to send your student’s transcript to any colleges and universities to which they are applying.

*Both student & parent have to sign under their

portal account & submit.
